When you pass your test, your examiner keeps the privisional and sends it off for you to recieve your full licence.
The when you recieve your full licence, you pay £20 at the post office and change your photo.
To claim your test pass by post with a photocard driving licence complete the declaration on your test pass certificate and send it with both parts of your licence (photocard and counterpart document) to DVLA, Swansea, SA99 1BJ
Personally I'd take photocopies of everything you send to DVLA, just in case.
Send it off with a full licence application form at the post office with your passport photo to DVLA Swansea (look it up on Google)
You must run around the island whilst hopping on one foot, and upon your arrival declare "I've made it."
