Because of the barriers. Barriers can interfere to your communication and it can lead to misunderstanding. Example of barriers is noise that surrounds you and your colleague.

Maybe it's because of wrong interpretation or having difficulties of trying to focus.
Misunderstanding happens when the speaker doesn't speak seriously or he/she speaks quietly or loudly. It is based on the tune of his/her voice, when he/ she speaks loudly, you'll think of him/her being mad, when he/she speaks quietly, you'll never hear what he/she speaks.
